Required Forms

To receive credit for completion of the Public Service Requirement, students must complete all required forms and submit them to the Feinstein Institute by the required deadlines. Forms must be completed for each public service placement or project that the student intends to apply to the 50 hour requirement. (NOTE: students may apply no more than 2 different public service placements or projects and must complete at least 20 hours on each project for it qualify for the completion of the requirement).

Once all forms have been received by the Feinstein Institute showing that the 50 hours of qualifying public service has been completed, the student will receive notification that the requirement has been fulfilled. The Office of Student Services will also receive certification of the student’s completion of the requirement.
