You must be admitted to the school you wish to visit and have approval from the Associate Dean of Academic Affairs before completing this agreement. You and the school you are visiting must complete this form together with their cost of attendance. Return the completed form to us so that we can complete your package based on this information.

Entrance/Exit Interview Form
To be completed by incoming first year students (only need to complete up to Student Signature) and at the end of studies upon graduation. Graduating students must also complete the Personal References section.
